WRITE THE PHARMACY IT MUST BE SENT TO (LIST ALSO ON TABLE).
DO NOT COLLECT FROM THE SURGERY
ALLOW 4 WORKING DAYS FOR PROCESSING.
POST REPEAT PRESCRIPTION REQUESTS THROUGH THE LETTERBOX IN THE DOOR TO THE LEFT OF THE MAIN ENTRANCE.
POST HOSPITAL LETTERS ETC THROUGH THIS LETTERBOX TOO.

Access
Inchpark Surgery has suitable access for disabled patients. A hearing loop is in place at the reception desk. There is a disabled parking space located at the front entrance of the building.
