NEW PATIENT REGISTRATIONS

We are registering new patients.  In the first instance please contact reception on 0131 666 2121 to check you live within the practice area. A patient registration form will then be sent to you.

Due to the high volume of new patient registrations we are receiving it is important that you call 0131 666 2121 to make an appointment prior to returning your form. Do not attend the surgery without an appointment to register as you will be asked to leave and come back once you have an appointment.

 

When you come to your appointment please ensure you return the form in person along with your proof of address (which must be valid for the previous three months) and photo ID. Please be advised we cannot accept a driving license as proof of address if using as photo ID and vice versa. The two forms of ID must be different and we will not be able to accept your registration form if this is not provided at the same time.

 

We do not require photo ID or proof address for persons under the age of 16 years old.

Community Link Workers  -  Tuesdays

A new service at Inchpark Surgery is now available with our community link workers who can help to link you with a wide range of local community resources and activities.  Please make an appointment at reception.

They can signpost you onto services related to:

Finance / Housing / Adult Literacy / Health Eating / Exercise / Volunteering / Substance Misuse / Mental Health / Local Groups & Activities / Employment / Domestic Abuse / Foodbank Referral / Carer Advice Coffee Mornings / Arts & Crafts / Sports Clubs / Children's Activities / Gardening Groups

Access

Inchpark Surgery has suitable access for disabled patients. A hearing loop is in place at the reception desk. There is a disabled parking space located at the front entrance of the building.
