New Patient Registration
When you come to register please ensure you return the form in person along with your proof of address (which must be valid for the previous three months) and photo ID (e.g. passport, utility bill, NHS medical card if you have one) for all people over 16 years registering with you. Please be advised we cannot accept a driving license as proof of address if using as photo ID and vice versa. We do not require photo ID or proof address for persons under the age of 16 years old. Patients who come from outwith the European Union may be asked to provide additional forms of information.
If you move out of the practice area, you will need to register with a new practice as soon as you move.
Please bring photo ID and proof of address for over 16s.
Preference of Practitioner
Patients are registered with the practice, not an individual GP. You can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. This preference will be recorded in your medical records and we will do our best to respect your choice. However, not all doctors in the practice provide all the services and specific doctors may not be immediately available.

Access
Inchpark Surgery has suitable access for disabled patients. A hearing loop is in place at the reception desk. There is a disabled parking space located at the front entrance of the building.
